We value your referral to Tweddle Child and Family Health Service. Your referrals will be triaged as usual by Tweddle’s Duty Worker and your client’s referral will be triaged into low, medium or high risk categories.
After assessment by the clinician your client will be placed on the standby list. Clients with medium to high risk will be offered a place in a program if there is a vacancy.
If your client meets the criteria they may be offered a Day Stay Program to commence the change process.

Families referred to Tweddle and triaged for a residential or day stay program will be supported by highly skilled and experienced Tweddle clinical psychologists, maternal & child health nurses, early childhood professionals and Social Support staff. Our services and programs are accredited, accountable, responsive to research and evidence based. These hallmarks of professional integrity are subject to national accreditation.
You may download our referral form and fax it to us (03) 9689 1922 or complete our online referral form. We will then contact the family for an intake assessment and advise you of progress and outcomes.
